When contacting Amazing LOHAS or submitting an Information Form, you should provide information that is accurate and complete to the best of your knowledge.
Relevant information may be shared with Independent Medical Providers for the purpose of coordinating an inquiry, consultation, or requested service, as described in our Privacy Policy.
You should inform Amazing LOHAS and the relevant Independent Medical Provider of any material change in your medical condition or other information that may be relevant to an ongoing inquiry or treatment assessment.
The Site, our Information Form, WhatsApp, email, and other Amazing LOHAS communication channels are not intended for medical emergencies or urgent medical care.
